What You'll Need to Enroll
- Proof of residency (utility bill, lease, or mortgage statement)
- Proof of age/identity (birth certificate, passport, green card, or other official record)
- Immunization record
- Health exam (required for Pre-K and Kindergarten)
- Completed enrollment form or packet
- Previous school records (if transferring)
- Custody or guardianship papers (if applicable)
Health, dental, and vision screenings must be completed within 45 days of entry for Kindergarten and Pre-K students.

Sports and Extracurricular Activities
WVSSAC Athletic InformationStudents participating in sports, band, cheer, or other extracurriculars must:
- Complete a WVSSAC Pre-Participation Physical Exam after May 1 each year
- Maintain academic eligibility
- Submit parental consent and emergency contact forms (given by school)
- Follow concussion and cardiac training requirements (given by individual school)
Additional Information
- Driver’s license eligibility in West Virginia is tied to school attendance and grades.
- Seniors must verify that their meningococcal booster is complete before graduation.
